Technology Related Forms

COSAM IT has developed forms that will assist the user with the delivery of Instructional Technology: 

• Instructional Software Request Forms are for faculty who want a particular software package in their class or to give students access to the software.
• Software Request Forms are for faculty/staff/GTAs who need to request software for individual use in their office.
• Waiver for Software Delivery Forms are for anyone who wants to physically install the software on their computer instead of electronic delivery.
