Auburn Medical Information Form
Upon arrival at Auburn University, and before you can officially register for classes, all new students must visit the Clearwave Web link and complete the Medical Information Form. Alabama state law requires you show written (and translated) proof of a negative tuberculin skin test. If you do not have proof of a negative test result, you will be required to take the test at the clinic. If the test results from your native country were positive, you are required to bring the x-ray film with you to the Auburn University Medical Clinic, along with a physician's verification (translated) that you are tuberculosis free.
General Information
Auburn University Medical Clinic is your primary care provider (PCP). AUMC is the first place you visit unless the clinic is closed.
A referral from AUMC is required for all specialist visits. If necessary, you must request a new referral each academic or policy year.
When AUMC is closed (after hours, certain weekend days and on major holidays or breaks), please visit a local urgent care.
When you have an emergency that requires immediate care, call 911 and use the East Alabama Medical Center Emergency Room.
If you are more than 50 miles outside of the Auburn area, you will need to find health care providers that are part of the Blue Cross Blue Shield of Alabama network. Women seeking maternity and gynecological services may go to the Women’s Clinic or a doctor in the Blue Cross Blue Shield of Alabama network without a referral - Provider search.
Children under the age of 18 years old will not be eligible to visit AUMC. Dependents under 18 on the student plan can obtain pediatric care from any in-network provider.
