Paper Application
 
If you are unable to use the online application, the  paper application is available for download. Instructions for completing the paper application may be found here. Print and complete the application and submit it along with the application fee of $40.00 to the Office of Admissions and Records, 108 Mary Martin Hall, Auburn University, AL 36849.
 
 
Additional Information Form
 
The online application does not include the Additional Information Form found on the paper application. This may be found by clicking the link and printing the form as instructed. You will need Adobe Acrobat 4.0 or higher in order to view. Please complete this form and mail it under separate cover to the address shown above.
 
 
Transfer Plan of Study
 
The information on this  transfer plan of study form will be used to help an Auburn University academic advisor in building your class schedule for your first term of enrollment. It is very important that the information you provide be accurate and complete if we are going to build you the best possible schedule. Students who are admitted and deposited before February 1 for summer or fall semester or before October 1 for spring semester will have a completed schedule with a minimum of 12 semester hours provided to them at their SOS (orientation) session. Please complete this form carefully and realistically. You will need Adobe Acrobat 4.0 or higher in order to view. Please keep a copy of this document for your records.
 
 
Medical Form
 
All students are required to complete the  medical information form prior to enrolling at Auburn. You will find more specific information regarding the medical form and general health care issues at  the AU Medical Clinic  website.
