All requests shall be evaluated against the appropriate project plan.
Each request shall require a justification to ensure its role in the appropriate project plan.
All problem reports should be initiated through the Incident form.
If a user calls and requests service, complete either an Incident form or a Request for Application Development/Maintenance form as appropriate.
All Requests for Application Development/Maintenance must be approved by the Manager of Applications Support.
One-time requests are to be filed sequentially by request number.
All other requests and incident reports are to be filed with the documentation of the appropriate job. One-time requests are to be filed sequentially by request number.
