This is a reminder that your AU Medical Information Form is required for you to start classes for the fall term of 2007. You may have already submitted the form, but if not, please do so prior to August 14. Please know that if your form is not received by that date in the University Medical Clinic, you may not be allowed
to enroll.

You can find the medical form by clicking here. Then you can complete it and mail it to 400 Lem Morrison Drive, Auburn University, AL 36849 or fax it to 334-844-6126, or even SCAN IT and EMAIL it to vickie.gunn@eamc.org.
