Please complete the relevant section(s) of the form (a file named "info.tex") and email the entire file to:

ajm@math.auburn.edu

If you wish to ``pretty-print'' the information you are providing here, you need to put the completed form into a file named "info.tex", then download the file ``pprint.tex'', then tex and postscript the latter.
