Summer Camp Forms

Participants in the following camps will need to complete and return the seven listed forms as well as proof of health insurance prior to the beginning of camp.  If the camp you are attending is not listed, you are not required to submit the forms.

Forms and Proof of Health Insurance

Complete and return the six listed forms as well as proof of health insurance prior to the beginning of camp. The following documents are provided in PDF format. Adobe Acrobat is needed to view the files. If necessary, download the free Adobe Acrobat Reader.

  4. Medical Release
  5. Medical Authorization
  6. Photo Release Form
  7. Transportation Form
  8. Proof of Health Insurance - You must send a copy of the front and back of your insurance card.  If you do not have health insurance, please contact James Birdsong, 334-844-5817 or james.birdsong@auburn.edu as soon as possible.
