Instructions for accessing/downloading the different file formats

Direct Web Printout
This form is very simple to access and anyone with a printer should be able to print one out. No downloading is necessary. Once you have the form displayed in your browser, simply click the "Print" option on the browser and the form should print out. Although not as fancy as the other forms, these forms are fully functional and include instructions.

PDF Files
PDF stands for "Portable Document Format" and is a popular file format used by Adobe. The primary advantage of .PDF files is that they retain all of the same formatting and graphics that are on the original form. If you do not already have one, you can use this link to get a Free Adobe Acrobat Reader for your computer. After you have downloaded the reader just follow the easy instructions included with the software to install Acrobat Reader on your computer. When you install the current version of Acrobat Reader it also automatically installs itself as a plug-in/activeX Control that should run when you use your Web browser (Netscape or Internet Explorer) to view .PDF files. If you have any problem getting Acrobat Reader to work as a plug-in you can always just save the file to your computer and read it manually with Acrobat Reader. It doesn't really matter if you download our .PDF file or Acrobat Reader first, but you must have both, with Acrobat Reader installed, to print out the .PDF forms. After you have the file downloaded and running in Acrobat, simply print it out on your printer, fill it out by typing or printing in ink, and return it to us.
