Office of the Registrar - Diploma Application Form

To ensure that your diploma is ordered with the name that you prefer, please complete your diploma application form by the 15th class day of the semester that you plan to graduate.

If your diploma application form is not submitted to the Office of the Registrar by the 15th class day, then the Graduation Section will order your diploma with the name that appears on your student record in Tiger i.

If you have checked the confidentiality flag in Tiger i, you must sign and date your diploma application form in order to release your name to be printed in the commencement program and give permission for your name to be announced from the platform at graduation.

If you have not already completed a Diploma Application Form, please obtain a form at the appropriate location listed below.

Undergraduate students should see the Student Services/Academic Affairs section of your Dean's Office
