COI Disclosure Checklist

Getting Started

    1. Log-in to the Endeavor system using your Auburn single sign on.
    2. Navigate to the COI tab.
    3. Select your Disclosure Profile.
    4. Click "Edit Disclosure Profile."
    5. Read the Instructions and Policies. Check the box to acknowledge. Click "Continue."
    6. Follow the prompts to answer the questions (sub-parts to some questions may be asked to obtain the full nature of the activity or relationship)
      1. Entity Disclosures: Disclose any financial interests or outside activities with outside entities including:
        • foreign interests/collaborations
        • equity
        • outside employment
        • consulting/professional services
        • board service
        • entities that do business with the institution
        • authored works used in the classroom
        • editorial services
        • intellectual property
        • divestiture/cash payout
        • paid travel
        • other appointments
        • gifts/honoraria
        • foreign interests
      2. Relationship Disclosures: Disclose any immediate family members working for Auburn University, AUM, ACES, or AAES
      3. Other Interests Disclosures: Disclose any other financial, romantic, or familial relationships that could appear to affect administrative decisions or the design, conduct, or reporting of research; or any other perceived conflicts of interest/commitment.
      7. Click ''Complete Disclosure Profile Update'' when your answers are complete and click ''OK'' to verify that all disclosure information is accurate and current.
The statements below are the types of questions you should ask yourself when completing your Disclosure Profile. These are simply guidelines to assist you in responding to the questions and do not constitute an all-inclusive list. You may have other financial interests or personal relationships not posed here that you should disclose in order to fully and truthfully complete your Disclosure Profile. Please read the Conflict of Interest Policy, and if an AU researcher, also the Financial Conflict of Interest Policy for Research and Related Activities, for specific requirements.

1. Entity Disclosures

For any Entity that is reasonably related to your Auburn University (AU, AUM, ACES, AAES) role(s)/responsibilities OR does business with (contracts, funds, licenses, provides goods or services to) OR sponsors research at Auburn University (AU, AUM, ACES, or AAES) ("You" means you, your spouse, your dependent child):
  • Do you have any interest or relationship with a non-U.S./foreign institution, foreign government, foreign company, or foreign national such as agreements, research collaborations, in-kind support, talent programs, visiting scholars, etc.? 
  • Do you have an equity interest such as stock, stock options, or own a company/business?
  • Do you engage in consulting or other professional services such as scientific advisor or expert witness?
  • Do you serve as a board member, paid or unpaid?
  • Do you engage in outside employment or activity such as a side/second job, self-employment, or sell goods or services?
  • Do you have a financial interest in or relationship with a company or organization that does business with the University?
  • Do you use published works that you have authored in your courses?
  • Do you provide editorial services such as a journal editor or grant reviewer?
  • Do you have intellectual property rights, income, or royalties?
  • Have you divested any equity or received a cash payout?
  • Have you received compensation or reimbursement for travel form a source other than Auburn University?
  • Do you have other appointments such as adjunct professor, honorary faculty, guest lecturer, fellow, etc.?
  • Have you received a gift or honoraria such as awards or speaking compensation over $1,000, or received any gifts from vendors or sponsors?

2. Family working at Auburn

  • Does your spouse work at Auburn University (AU, AUM, ACES, or AAES)?
  • Do any of your children, stepchildren, or grandchildren (dependent or adult) work at Auburn?
  • Do any of your siblings work at Auburn?
  • Do any of your aunts/uncles, cousins, nieces/nephews work at Auburn?
  • Do any of your parents or grandparents (including step) work at Auburn?
  • If you have immediate family working for Auburn University, do you engage research or any other university activities with this individual?

3. Other Interests that could affect administrative decisions

  • Are you dating someone whose company has put in a bid for a Request for Proposal and you are on the committee reviewing the bids?
  • Are you on the hiring committee for a position for which your best friend or sorority sister is applying?
  • Are you the Primary Investigator (PI) on a research grant where your relative is also on the project?
  • Is your child a student in the same program or department in which your work?
  • Are you close family friends with a company that does business with Auburn and you are involved in making purchasing or contract decisions for Auburn with this company?
  • Do you have any former financial interests or relationships that might affect your decision making or performance of your institutional responsibilities?
  • Do you serve on any other boards, commissions, our councils on a voluntary basis or that are unrelated to your institutional responsibilities?

4. Update Your Disclosure

  • Have you acquired any new interests or relationships since your annual disclosure?
  • Have you started a business, received a new consulting opportunity, or been hired to work for another entity?
  • Have you started a romantic relationship with another employee or a vendor?
  • Has your child or another relative started school or employment at Auburn?
You may log in and revise your answers to make corrections or additions at any time. Simply log into aub.ie/endeavor and Edit your Disclosure Profile.
