e. Disclose any other personal relationships (with non-university individuals) that could affect administrative decisions
f. Disclose any other potential conflicts of interest/commitment for transparency
g. Disclose whether you or not you currently have a COI management plan

The statements below are the types of questions you should ask yourself when answering the questionnaire. These are simply guidelines to assist you in responding to the questions and do not constitute an all-inclusive list. You may have other financial interests or personal relationships not posed here that you should disclose in order to fully and truthfully complete the questionnaire. Please read the Conflict of Interest Policy, and if an AU investigator, also the Financial Conflict of Interest Policy for Research and Related Activities, for specific requirements.
