Why don't the hyperlinks in my PM form work?

How to fill in the page headers on my PM form? 1) fill in the fields for employee name and rating period, then 2) click Office Button > Print or Office Button > Print > Print Preview

* How to enable macros in MS Word 2007: Click "Options" button in the information bar...

Then select "Enable this content" radio button and click "OK"...
