AU MAMMOGRAM DAY

November 18th, 2009
7:30 a.m. - 3:30 p.m.

Who is covered: employees and spouses covered under the university's Blue Cross Blue Shield insurance program. Participants are eligible if they are 35 or older, have never had a mammogram or have not had a mammogram in the last two years.

68 spots available by appointment on a first-come, first-served basis.

Not recommended for those who have implants.

Fred Kam of the Auburn University Medical Clinic will be the referring doctor; upon request, results can be sent to your individual doctor.

Those who have already scheduled a yearly mammogram are encouraged to keep their appointment and allow the spaces during Mammogram Day to be used by those who are overdue.

Auburn's insurance provides for an annual breast exam and mammogram.

Time off will be allowed for the initial exam, and, if needed, the first round of additional testing. If further testing is recommended, the employee will be required to use sick leave.

When making the appointment, please have the following information available: Blue Cross Blue Shield number, date of birth, date of last mammogram, Social Security number, phone number and the policy holder's name.

If the mammogram detects a possible problem and additional tests are required, employees will be required to pay the co-pay and deductibles related to the additional testing.

To make an appointment, call Colleen Alsobrook of East Alabama Medical Clinic at 528-4370 and mention you are participating in the Auburn University Mammogram Day. Mammograms will be done at Auburn Diagnostic, 1527 Professional Parkway.

For more information, contact Jenny Swaim at 703-4025 or swaimje@auburn.edu.
