SPECIMEN LOAN REQUEST FORM
Reptile and Amphibian Division
LOAN POLICY
- Loans will be made for a period of six months. Requests for extensions should be directed to the Curator.
- Loan requests from students must be co-signed by a faculty member from the borrowing institutuon.
- A copy of the loan invoice must be signed and returned to the Curator upon receipt of the specimens.
- All material borrowed must be properly housed and cared for at the borrowing institution. Alcohol specimens must be kept in 70% alcohol. Special specimen care instructions indicated on the loan form must be precisely followed. Written approval of the Curator must be procured for alternative storage and/or housing arrangements, and for any uses that will result in permanent alteration of any specimen.
