SPECIMEN LOAN REQUEST FORM
Reptile and Amphibian Division

LOAN POLICY

  1. Loans will be made for a period of six months. Requests for extensions should be directed to the Curator.
  2. Loan requests from students must be co-signed by a faculty member from the borrowing institutuon.
  3. A copy of the loan invoice must be signed and returned to the Curator upon receipt of the specimens.
  4. All material borrowed must be properly housed and cared for at the borrowing institution. Alcohol specimens must be kept in 70% alcohol. Special specimen care instructions indicated on the loan form must be precisely followed. Written approval of the Curator must be procured for alternative storage and/or housing arrangements, and for any uses that will result in permanent alteration of any specimen.
