- Have the publication information for each textbook (including the
ISBN and edition). Each blank must be completed before you can submit
the form.
- Select the appropriate form.
- Complete the form and submit it. You will receive an acknowledgement
if the form is submitted successfully.
- Repeat the form for each book. (We know this is somewhat cumbersome,
but we are still learning how to do this.)
