NOTICE: This form will not automatically be submitted electronically. However, you may attach it to an email and send it to intledu@auburn.edu. Title the email: Auburn Abroad Application. Lastly, keep a copy of this Auburn Abroad Package for your files!

This release must be signed by students, staff, faculty who participate in study/intern/work abroad programs in countries with United States Department of State Travel Warnings in effect. These forms will be sent to the provost for final approval.

FORM A:     Student AA Application form

Form needed to get you accepted into the Auburn Abroad (AA) Program, whether you are taking graded or transfer courses.  AA Requirements are listed on this form for easy review.

 
FOR AUBURN STUDENTS ONLY

FORM B:    Student AA Course Approval Form (CAF)

Form needed to enroll you in the correct section of Auburn Abroad.     Enrollment in the AA "course" means that you are considered full time at AU while abroad, regardless of the number of credit hours you are taking abroad. This form is also used by Financial Aid and the PACT program.

NOTES:    1. Upon completion of this form, with all signatures, courses listed and their AU equivalents,  are officially pre-approved for transfer back to your AU transcript at the end of your abroad program.

2.  Any courses you take abroad that have not been pre-approved on this form may not transfer back as AU equivalents.

3. Unsure of which courses to take?  List more courses (with title and course number), than you will need.  Email any course changes to AU OIE and your Academic Advisors as soon as you make these changes, while you are still abroad.  This will avoid confusion and delays in posting your grades to your transcript at the end of your program.

This form details many aspects of the abroad experience such as registration for Auburn Abroad, refund policies, prohibitions regarding motor vehicle operation,  requirements to attend AU pre-departure sessions, refund policies, etc.

This form details unacceptable behavior and the consequences.  We know most students will be terrific ambassadors for AU, however, you are required to sign this for so that you can't say that we didn't warn you about certain behaviors which would decrease the effectiveness and enjoyment of all program participants.

This form gives us your contact info abroad and your stateside emergency contacts. This is need in case of emergency at home or abroad.  Additionally, the top of the form gives you directions to register yourself at the US Embassy nearest to your study or internship site.  If you have trouble registering, we can help.

NOTE: if your contact info abroad changes, email us ASAP  so we can change your Emergency Contact Info.  You can email us at  intledu@auburn.edu , put "AUBURN ABROAD Contact Info" in the subject line.

Use this to provide feedback after the end of your program.  Include the good and the bad, continue on another page if you need to provide more comments.  Your comments are invaluable.  Used anonymously, they will help prospective  students get the "insider's view" of a program.

Financial Forms

FORM G1: Student Financial Aid Budget Form

Used by Financial Aid to determine appropriate amount of funds to award to student.

NOTES:  (1) Apply for Financial Aid early.  (2) Include all of your expenses.  See COMMENTS section of this budget to insure that you are not missing anything.  Under-estimating expenses could cause you to run out of funds prior to the end of the program.  (3) Work with the AU Financial Aid Officer to insure that the timing of funds works with your program payment schedule.

 

FORM G2: Student Financial Aid Contractual Agreement Form

Required by AU Financial Aid Office in order to comply with US Department of Education Financial Aid Requirements. 

ALERT:  This form is used by students on Non-AU Programs and must be sent to the Non-AU program (usually the Program Director listed on your program application) for signature.  Do this early to give the Non-AU program director time to respond.  

Students on AU Programs will not use this form.  Instead, they should check with their AU Program Director to obtain a copy of the AU Program Contractual Agreement and forward this copy to Financial Aid and the AU OIE office.

PACT Certification Letter Download:  Word - Form Fill

Students receiving PACT funds must fill out this form in order to receive funds for the time period studying abroad.
