Abstract

Database development to track individuals with disabilities and to develop new services in North Carolina county service area. The project will: a) identify consumers by age 14, b) educate the county area about services, c) identify new needs or services, and d) estimate annual budgets based on tracking.  

The education portion of the project will include a quarterly transition fair that will allow residents to learn about services in the area, as well as provide participants with a community resource manual.
