Technology Related Forms

COSAM IT has developed forms that will assist the user with the delivery of Instructional Technology.  

• Instructional Software Request form is used by faculty who what to use a particular software package in their class or to give students access to the software.
• Software Request is for faculty/staff/GTAs to request software for individual use in their office
• Waiver for Software Delivery is for those who want to physically install the software on their computer instead of electronic delivery
